More Time to Heal. More Support for a Safe Return Home: How Swing Bed Care Supports Recovery After a Hospital Stay

Swing bed care is short-term, hospital-based care for patients who are well enough to leave acute hospital care but are not yet ready to return home safely. It can be for patients after surgery or after a hospitalization for an acute condition like pneumonia.

Also called post-hospitalization recovery, swing bed care provides skilled nursing, rehabilitation, and continued medical support while a patient regains strength and prepares for the next stage of recovery.

At Plains Memorial Hospital, our Swing Bed program allows eligible patients to continue recovering in Dimmitt, close to their families, caregivers, and community.

What Is Swing Bed Care?

“Swing bed” may sound like a funny name for an important healthcare service. The term simply means that a hospital can re-categorize a hospital bed from acute inpatient care to short-term skilled recovery care based on a patient’s needs.

During an acute hospital stay, the care team focuses on treating a serious illness, injury, or medical condition or providing post-surgical care. Once that intensive treatment is no longer needed, some patients may still require nursing care, physical or occupational therapy, wound care, medication support, or help rebuilding their strength and re-establishing activities of daily living.

Who May Benefit From Post-Hospitalization Recovery?

Swing bed care may be appropriate for a patient who no longer needs acute hospital treatment but still needs additional care before returning home.

Patients may benefit from swing bed care while recovering from:

  • Surgery
  • A serious illness or infection, such as pneumonia
  • An injury
  • A long hospital stay
  • Weakness or reduced mobility
  • Wounds that require continued care
  • A condition requiring IV therapy
  • A health event that makes daily activities more difficult

Every patient is different. A hospital care team can evaluate the patient’s medical needs, recovery goals, and ability to safely return home and make a determination about post-hospitalization care and whether swing bed care is appropriate.

What Services Are Available?

The services included in a swing bed stay depend on the patient’s individual needs. Care at Plains Memorial Hospital may include:

  • Skilled nursing care
  • IV therapy
  • Sterile dressing changes
  • Skin and wound care
  • Medication management
  • Physical therapy
  • Occupational therapy
  • Palliative care
  • Assistance with daily activities
  • Care coordination and discharge planning

Each patient receives a personalized care plan designed to support recovery, improve function, and prepare for a safe transition home or to the next level of care.

Can Patients Return to Dimmitt for Recovery?

In many cases, yes.

Patients who receive surgery or complex hospital care in Amarillo, Lubbock, or another community may be able to transfer to Plains Memorial Hospital for continued recovery closer to home.

Returning to Dimmitt can make it easier for loved ones and caregivers to visit, participate in discharge planning, and provide support during the recovery process.

A transfer must be medically appropriate, and eligibility may depend on the patient’s condition, care needs, and insurance coverage. The hospital care team and our Swing Bed Coordinator can help review the available options.

What Happens During a Swing Bed Stay?

The process generally includes three steps:

1. Evaluation for Skilled Care

The care team reviews the patient’s condition and determines whether additional skilled nursing or rehabilitation services are needed.

2. Admission and Care Coordination

Once the patient is approved for swing bed care, the team develops an individualized recovery plan. This may include nursing care, therapy, medication support, wound care, and other services.

3. Recovery and Discharge Planning

The care team monitors the patient’s progress and prepares the patient and family for what comes next. Before discharge, the team may review medications, follow-up appointments, home instructions, equipment needs, and additional services.

The average swing bed recovery is one to two weeks, but the length varies depending on the patient’s medical needs, progress, recovery goals, and discharge readiness.

What Is the Goal of Swing Bed Care?

The goal is to help patients recover safely and become as strong and independent as possible before leaving the hospital.

For some patients, that may mean rebuilding strength and mobility through therapy. For others, it may mean completing IV treatment, receiving wound care, or learning how to safely manage medications and daily activities.

If a patient’s condition changes and acute hospital care is needed again, the level of care may be adjusted without requiring the patient to move to a different facility.
